If you are staying for a week, definitely yes. If you are there for just a couple nights, you'll be fine with a taxi ride to and from the airport, and then travelling on foot, river taxi, or tour bus the rest of the time.
I would say definitely no to a car. My wife and I were there for a few nights last fall and had no need for one at all. We stayed at the Menger which is walking distance from the Riverwalk, Alamo, La Villita and even Market Square (although that is a long walk).
If you want to visit the missions, there is a tour bus that leaves from the Menger and runs to all the other attractions every hour. You can get a pass that lets you on and off all day (or for a few days). Shuttles from the airport are easy and fairly cheap.
Remember though that there won't be many grocery options without a car. I'm not aware of any full grocery stores that are walkable from the Riverwalk. The only place with basic groceries that I can think of right now is the CVS drug store at the Riverwalk.
We spent 5 nights on the River Walk over Thanksgiving a couple years back. We stayed at the Homewood Suites, and I can't remember exactly where, but we were able to get some basic groceries within walking distance for our breakfasts / light brunches. As for dinner, there were so many amazing restaurants we didn't even think about cooking.
We had no problems filling up five days with activities within walking distance, so no car was needed for us.
