I've used eFax in the past. I've had a free account for over a decade... maybe close to two. I haven't used it in a few years though... so not sure of the status.
We no longer have fax available on our computers, but have been able to request PDF's via email as many copiers will email as well as fax. I would check with the staff at the doctor's office if that is a possibility.
I use efax as well
But if all you need is the pdf form then. I use an app called TurboScan
The sender takes pics of the document and sends the via email or whatever in pdf
